I guess it should be an experiment related to reproduction. The mother reproduces cells through the night common, and then grows them in the abdominal cavity of the boy.
During the breeding period in the hospital, the nurse who took care of Nicola had seen Nicola's paintings and perhaps remembered the outside world. After hard work, she helped Nikola escape the test island, and the nurse, who knew she could not survive, chose to commit suicide by jumping into the sea after successfully escaping. And what Nikola will face is a new world.
Let’s take a closer look. The boy Nicola saw on the bottom of the sea has a starfish in his abdomen. The starfish is a carnivorous marine animal. According to the content at the back of the movie, the boy’s abdomen has undergone surgery. Why did the drowned boy try to escape? It can be seen that the women are not sure where the boy is, and they would not throw the boy's body into the sea, otherwise they would not be so surprised. At the same time, when they salvaged the boy's body at night, they lifted the blanket and saw the hair, confirming the boy's identity, which shows that this was an accident.
Let's look at Nikola's awakening. Nikola saw the boy's corpse on the bottom of the sea, and his self-consciousness began to awaken. In his worldview before, everything that happened was normal until he saw the corpse. When he eats strange foods and drugs every day, he will associate himself with the boy. The cars, ferris wheels, and tall buildings he painted are obviously not what this world should exist. Then it can only be the consciousness in his brain when he was very young.
When Nikola entered the hospital, it further confirmed his own imagination that the boys under the sea are the same kind of people as them. The starfish on his stomach gave him a deeper understanding of his environment. His consciousness gradually awakened, and he was awake and chaotic from time to time. In fact, all of this happened in real, but the doctor used drugs to affect him, making certain contents look like dreams.
When the female nurse saw Nicola's painting, her face was more innocent. Those paintings inspired her desire for the outside world. She and Nikola are lingering under the water, exchanging air to keep swimming underwater for a long time, and at the same time it is like a ritual, a ritual of a man and a woman. In short, only through this way can she return to the outside world. In the end, she took the boy out and jumped back into the sea. What was waiting for her? Maybe it's death, maybe it's returning to the original island, who knows?
