I think Anna is actually better. When she got out of the mental hospital, it was because her dad had an affair with the nurse that caused her anger but accidentally burned her mom and sister to death. So when she got out of the hospital, she had to finish unfinished things, kill the nurse and go home. Anna pretended to be a mentally ill person and planned everything (in the mental hospital, she was the nanny who killed three children and a pearl necklace. She wrote, directed and acted by herself. After completing the murder, she was caught by the police, and she could return to the mental hospital safely. The last scene Anna's smile at the babysitter door in the mental hospital I wonder if everyone agrees with me)
