Who I am is important, Reivew on Unknown

When one day you wake up and find that your identity has been questioned, until you find that another person has completely taken your place, you make excuses, but no one believes you, and finally you doubt yourself and who you really are... There
is one point in the movie that I don't quite understand. Was the male protagonist's car accident premeditated or an accident? If it was a premeditated plan, its purpose must be to replace the male protagonist's current doctor's identity with another person. If it was an accident, why did the killer group later hunt down the male protagonist who was originally a killer?
